Position Overview

Advanced Practice Nurse (APN) – Delir / Kognitive Einschränkungen (m/w/d) at Vivantes Netzwerk für Gesundheit GmbH, Auguste-Viktoria Klinikum, Berlin. The role can be undertaken on a full‑time or part‑time basis (morning, afternoon, evening). Employment is open‑ended.

Responsibilities

• Optimisation of care for a specific patient group through development and implementation of patient pathways, care concepts, and nursing models.
• Service development using evidence‑based knowledge and best‑practice models (e.g., Safewards, clinical quality standards).
• Active participation in strategic decisions shaping nursing at the site.
• Conducting case discussions and multidisciplinary conferences.
• Training, advising, and guiding patients, relatives, and nursing teams.
• Design and continuous improvement of training programs, workshops, and onboarding concepts in alignment with internal offerings.
• Literature research and preparation of scientific findings for clinical implementation.
• Evaluation of existing care situations using qualitative and quantitative data, and derivation of improvement measures.
• Establishment of inter‑disciplinary advisory structures (e.g., collegial care with nurses, physicians, therapists).
• Building and maintaining systematic knowledge management and promoting knowledge transfer.
• Working 20 % in direct patient care and 80 % in conceptual and developmental roles.

Qualifications

• Completed nursing education and a master’s degree in nursing science.
• Several years of professional experience, particularly in complex care situations.
• Experience in project and quality management.
• Knowledge of qualitative and quantitative analysis methods.
• Proficient use of standard MS Office applications.
• Strong sense of responsibility, teamwork, and patient orientation.
• Commitment to evidence‑based and quality‑oriented work.
• Excellent communication skills.
• Active engagement in nursing and inter‑professional education.
• Strong language and text comprehension as well as analytical thinking.
• High quality and service orientation.
• Ability to work independently, structured, and solution‑focused.
• Empathy and respectful communication skills.
